Medicare-eligible nutrition care through referral

If you have been referred by your GP or specialist, I provide personalised, evidence-based nutrition care to support the management of chronic health conditions. Clients referred under a Medicare Chronic Disease Management (CDM) plan or another eligible pathway may be able to access Medicare rebates for dietitian consultations.

How your dietitian consultations work

Initial consultation (60 minutes)

  • A comprehensive nutrition assessment
  • A review of your medical history, medications and referral information
  • A discussion of your current eating patterns, challenges and goals
  • Identification of your key nutrition priorities
  • Personalised nutrition goals and strategies

Follow-up consultations (30 to 45 minutes)

  • Monitoring progress and evaluating outcomes
  • Reviewing and adjusting your nutrition strategies
  • Ongoing evidence-based nutrition education
  • Support for behaviour change and long-term success

Common referral reasons for a dietitian

I provide nutrition support for a range of chronic health conditions and nutrition concerns, including:

Payment and Medicare rebates

  • Consultation fees are payable on the day of your appointment
  • Eligible Medicare rebates can be claimed after your consultation
  • Any remaining out-of-pocket cost (gap fee) is your responsibility

A valid referral from your GP or specialist is required to claim a Medicare rebate. Your referral allows me to work alongside your healthcare team to provide personalised nutrition care.
